Mission & History

Why we started coordinating care differently

Accountable Care Physicians Group was formed by a group of Florida physicians who kept seeing the same problem: patients bouncing between specialists with no one connecting the dots. We built ACPG to close that gap — pairing primary care physicians with specialists and ancillary providers under a shared model of accountability for the Medicare patients we serve.

Since then, our network has grown across dozens of Florida counties, but the standard hasn't changed: better communication, better documentation, and better outcomes, measured openly.
